License Agreement
The user is granted permission to produce printouts of the Patient and Caregiver Information Sheets to hand out to their patients in the course of their practice work. Producing printouts and storing a backup copy of the Patient and Caregiver Information Sheets on another device is permitted for personal practice use only.
Other than as stated in this License Agreement, you may not copy, print, modify, remove, delete, augment, add to, publish, transmit, sell, resell, create derivative works from, or in any way exploit any of the Patient and Caregiver Information Sheets’ content, in whole or in part, and you may not aid or permit others to do so.
You shall not:
- rent, assign, timeshare, distribute, or transfer all or part of the Patient and Caregiver Information Sheets or any rights granted by this License Agreement to any other person;
- duplicate the Patient and Caregiver Information Sheets, except for practice use and for reasonable backup copies;
- remove any proprietary or copyright notices, digital watermarks, labels, or other marks from the Patient and Caregiver Information Sheets or their contents;
- transfer or sublicense title to the Patient and Caregiver Information Sheets to any other party.
